Declaration of consent to the use of image

I, the undersigned, pursuant to Art. 81 sec. 1 of the Act of February 4, 1994 (i.e. Journal of Laws of 2021, item 1062, as amended) on copyright and related rights, I declare that I consent to the recording and free, multiple, dissemination of my image and scientific materials through the use of photos, online transmission via the Internet, social media, intranet and internal communication of the organizer for the promotion and transmission of the scientific conference entitled ATAM & MASCA 2024 organized on 05-07.06.2024 organized by RESEARCH NETWORK ŁUKASIEWICZ – PORT POLISH CENTER FOR TECHNOLOGY DEVELOPMENT based in Wrocław, ul. Stabłowicka 147 and the publication of video material with my name and surname and scientific materials from the above-mentioned conference by the ŁUKASIEWICZ RESEARCH NETWORK – PORT POLISH CENTER FOR TECHNOLOGY DEVELOPMENT for promotional, advertising or educational purposes. At the same time, I declare that the above-mentioned video materials with my participation do not infringe my personal rights. This consent is not limited in time or territorial, applies to all materials provided by me for its promotion and materials with my participation made during the conference entitled ATAM & MASCA 2024 organized by RESEARCH NETWORK ŁUKASIEWICZ – PORT POLISH CENTER FOR TECHNOLOGY DEVELOPMENT. The image can be used for all kinds of forms of electronic image processing, framing and composition, with no obligation to accept the final product, but not in forms that are offensive or generally considered unethical.

Information clause

Pursuant to Art. 13 sec. 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016. on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC (General Data Protection Regulation) (Journal of Laws UE L 119 of 04/05/2016, p. 1), hereinafter referred to as “GDPR”, I would like to inform you that:

1. The administrator of personal data provided to the Ordering Party as part of this procedure (and for the purposes of concluding and performing the contract as a result of it) is (contact details): Network Research Łukasiewicz – PORT Polish Center for Technology Development with its seat in Wrocław, ul. 2. The administrator appointed the Data Protection Officer (“DPO”). Contact with the DPO: iod@port.lukasiewicz.gov.pl Please contact us in all matters relating to the processing of your data.
3. Your personal data provided in the registration form are processed in order to register participation in the conference entitled ATAM & MASCA 2024, organization and communication with participants on matters related to the organization of the conference and its promotion, and documenting its course. The legal basis for the processing of personal data is Art. 6 sec. 1 lit. a GDPR, i.e. consent. You can withdraw your consent at any time – which does not affect the lawfulness of the processing carried out before its withdrawal.
4. The personal data provided by you will not be disclosed to other entities, except for entities authorized by law, including those involved in the organization or service of conferences.
5. Personal data processed in connection with participation in the conference will be processed during the implementation of the purpose of processing, as well as for the periodDECLARATION OF CONSENT TO THE USE OF IMAGEof storing financial and accounting documentation of the conference by the university, in situations provided for by law, they may also be processed in the period necessary to establish and pursue any claims. Personal data will be processed until the consent is withdrawn or objection to data processing is expressed.
6. You have:
   • a) based on Article. 15 of the GDPR, the right to access your personal data;
   • b) based on Article. 16 GDPR, the right to rectify your personal data;
   • c) based on Article. 17 GDPR, the right to delete personal data in cases provided for by law;
   • d) based on Article. 18 GDPR, the right to request the administrator to limit the processing of personal data, subject to the cases referred to in art. 18 sec. 2 GDPR;
   • e) based on Article. 20 GDPR, the right to transfer personal data, in accordance with applicable law;
   • f) based on Article. 21 GDPR, the right to object to the processing of personal data in cases provided for by law;
   • g) the right to lodge a complaint to the President of the Personal Data Protection Office, if you feel that the processing of your personal data violates the provisions of the GDPR.
7. With regard to your personal data, decisions will not be made in an automated manner, in accordance with art. 22 GDPR, they will also not be profiled.
8. Providing your personal data by you and consenting to their processing and dissemination of your image is voluntary, and in the event of refusal, it will not be possible to participate in the conference.
